Our patients are our top priority. Whether you've visited us before or it's your first time at Clinic W, you can be assured that our staff will do everything they can to serve you well. Please remember to bring a form of identification and your insurance card during each visit.

For a shorter time in the waiting room, please fill out the New Patient Form and bring it with you to your appointment.

Accepted Insurance:

We accept all major insurances. Medicaid is only accepted as a secondary form of insurance. 

When you visit:

Please bring a current list of all of your medications.

Payment is due at the time of your visit. 

After you visit:

We understand that you are busy. We've created an online portal to allow our patients to access their health information online whenever they need it. At the end of your first visit, our staff will give you login credentials so that you can access your health information from anywhere, at anytime. If you already have your login credentials, then you can access our portal here.
